Blood tests when gluten free

How important is eating gluten before a blood test for celiac? And can I get away with eating it for just a week or so before? I’ve eaten gluten free for about a year now. I notice when I eat gluten I get mouth ulcers and large painful canker sores, and more recently welts and an itchy rash - the first time this was on my finger, and the second time I hadn’t even realised I’d eaten gluten but woke up with welts coming up around my ankle, which was extremely itchy. It was only upon looking at a baked beans can that my daughter had cooked for me the night before that I realised it had gluten. Also, since becoming more strict about not eating gluten in the past several weeks the soft stools I’d learned to expect as normal have - at least half of the time - firmed up. I stopped eating meat years ago due to chronic diarrhoea but now it seems I can eat meat again, also. I’ve always put my low iron down to my vegetarian diet (even though it went up when I stopped eating meat!). 
 

My doctor told me not to worry about eating gluten before a blood test and to treat it as celiac regardless, as I could get a false negative. But I’d rather have a formal diagnosis if that’s what it is, as I feel like an idiot saying I’m celiac when I’m not officially diagnosed (because people don’t take NCGS seriously, and I’m tired of explaining myself and what happens when I do eat gluten). I also have this perpetual doubt and think that each incident where this has happened was a coincidence.

Eating a lot of gluten daily for 6-8 weeks before a blood test is critical, and the results of the test won't be accurate if you do not do this.

Here is more info about how to do a gluten challenge for a celiac disease blood panel, or for an endoscopy:

  • Eat gluten prior to celiac disease blood tests: The amount and length of time can vary, but is somewhere between 2 slices of wheat bread daily for 6-8 weeks and 1/2 slice of wheat bread or 1 wheat cracker for 12 weeks 12 weeks;
  • Eat gluten prior to the endoscopic biopsy procedure: 2 slices of wheat bread daily for at least 2 weeks;

That seems to be an official diagnosis.

It really comes down to do you want to become unsick ? 

Tell them your doctor said it is Celiac so you are following his orders and eating gluten free.  Then challenge them to give up gluten for one month, just to prove they are not addicted to gluten.  if they are not addicted, it should be no problem.  Usually they will get mad.

Diagnosing Celiac is an art.  Some people are seronegative and biopsy positive, some seropositive and biopsy negative, some seronegative and biopsy negative but positive for Dermatitis Herpetiformis.  Follow your doctors advice.

Eventually your freinds will be getting sicker and you won't. Then, it may take years, but then you can say: 'Told ya!".

      Ty very helpful I’m in the process of getting in with a new gI doctor and will ask more about all that then and yes the rash is exactly like that. I have a half cousin with celiac but that’s it in my whole family and idk if that matters as I see it is hereditary and others say the the only in the family with it. Either way this was very much appreciated ty again.

      My first post includes the blood test report done by the second doctor.  They told me to take two Naproxen tablets per day.  Absolutely worthless but I'm doing it.  I get to visit the doctor again in two weeks.  These doctors are worthless.  I had to clue them in to the fact that they used to treat me for fibromyalgia 15 years ago with vicodin until some political stooge had it removed from the market.  Then I found out about my grandchild with celiac so I removed gluten from my diet and got better.  (I had hashimoto's then and still do).  They don't read my medical record, or they don't comprehend it.  Either case they're worthless turds with no answers that are helpful.

      Approximately 10x more people have non-celiac gluten sensitivity than have celiac disease, but there isn’t yet a test for NCGS. If your symptoms go away on a gluten-free diet it would likely signal NCGS.

      Have you been able to see a doctor or rheumatologist about this? Inflammation like that could point to something like celiac complications, an autoimmune response, or even reactive arthritis, and you deserve relief and answers. In the meantime, gentle heat/cold therapy or anti-inflammatory foods and over the counter meds might help a little, but I hope you can get proper medical support soon.

      None of your test results suggest celiac disease but your symptoms suggest either celiac or NCGS.  There is another test mode for celiac disease that might be pursued and that is an endoscopy with biopsy of the small bowel lining to check for the damage caused by celiac disease inflammation. This is the gold standard test for celiac disease and is usually done even when blood antibody test scores are positive. However, you might not find a physician willing to do the procedure in the absence of negative celiac antibody test scores. The absence of any positive celiac antibody test scores suggests that damage is not occurring to the lining of the small bowel. The endoscopy/biopsy would involve a referral to a GI doc. One question about your rash. Are there pustules in the bumps? This is characteristic of the rash associated with celiac disease known as dermatitis herpetiformis. My suggestion for you at this point would be to trial the gluten free diet for three months and see if your symptoms improve. If so, you likely have NCGS. If not, then you need to look in a different direction for answers to your health issues. If you do go on a gluten free diet, realize that you would need to go back on gluten for several weeks if you ever want to get tested for celiac disease again. Current guidelines for the "gluten challenge" recommend the daily consumption of at least 10g of gluten (the amount in about 4-6 slices of wheat bread) for at least two weeks prior to testing. But I would certainly give it more than two weeks to be certain of valid testing. I would also talk to your physician about your high immunoglobulin A Qn score. It may mean nothing but it can also be an indicator of some potentially serious health issues. You should google it. Here is a primer for getting off to a good start with eating gluten free:
